Documentation required for CRT work
After you have registered online as a CRT we will then contact you, when there is enough available work in your area. You will then be asked to attend a group interview/information session in which case you will be required to bring along certain documentation.

Prior to coming in for an interivew you will need to download and complete the following. Please try to print each form double sided:

- Tax File Number
- 100 points of ID (eg. *Certified copies of your Passport and driver's license)
- Resume with at least 2 referees
- A certified copy of your VIT card
- Certified copies of a Level 1 and/or 2 First Aid and Anaphalaxis certificate (Crts are not required to have this but are encouraged to do the training offered through ITS at a discounted rate)
* For a document to be certified the officer must write on every page of the document, 'I have sighted the original document and certify this to be a true copy of the original.' The person should then sign each statement and provide their designation, for example, 'Pharmacist'. Click here for a list of certifying officers
