Register for Out of Hours School Care

 

Please download and fill in the following documents and send to Inspired Teaching Solutions PTY LTD. PO Box 650, Richmond 3121

We also require the following

  • Tax File Number Declaration Form (Please get one from your local newsagency and fill out the top section. Please don't remove any of the carbon copies)
  • 100 points of ID (eg. *Certified copies of your Passport and driver's license)
  • Resume with at least 2 referees
  • A certified copy of your VIT card or Working With Children Check
  • Certified copies of a Level 1 and/or 2 First Aid and Anaphalaxis certificate (If you have it)

* For a document to be certified the officer must write on every page of the document, 'I have sighted the original document and certify this to be a true copy of the original.' The person should then sign each statement and provide their designation, for example, 'Pharmacist'. Click here for a list of certifying officers
